A BOC-3, also written as "BOC 3" or "boc3 form", is a Designation of Process Agents required by the FMCSA under 49 CFR Part 366. All for-hire motor carriers, freight brokers, and freight forwarders operating under interstate authority must have one on file before their MC or FF number can be activated.

Who Needs a BOC-3?
You need a BOC-3 filing if you're applying for or currently hold:
Motor carrier authority (MC number) for hauling freight or passengers
Broker authority (MC number) for arranging freight to be carried
Freight forwarder authority (FF Number) for break-bulk / consolidation of shipments
In summary, if you have an authority in addition to your DOT number, you need a BOC-3. Private carriers who only haul their own goods and anyone else that doesn’t have an MC or FF authority do not need a BOC-3 filing.
If you've already applied for authority, the FMCSA will automatically dismiss your application after 90 days if you haven't filed proof of insurance and a BOC-3. If your BOC-3 is removed from your authority for any reason, your authority will be revoked unless a replacement is filed.

Operating without a valid BOC-3 is a federal violation under 49 CFR §366. Fines can reach $16,000 or more per incident, and your operating authority can be revoked. Your authority cannot become active in the first place without a BOC-3 on file.

Go to the FMCSA's SAFER system at safer.fmcsa.dot.gov and search your USDOT number. Your BOC-3 status will appear under your authority record. If it shows no process agent on file, you need to file before your authority can be activated.
